IDENTIFICATION
[ ] Photo ID or driver's license for every adult
[ ] Birth certificates for children in the household
[ ] Social Security cards, if available
[ ] Immigration or residency documents, if applicable

INCOME AND BENEFITS
[ ] Pay stubs for the last 30 days
[ ] Social Security / SSI / SSDI award letter
[ ] TCA, SNAP, or other benefit award letters
[ ] Unemployment or pension statements
[ ] Child support documentation
[ ] Self-employment records or most recent tax return

HOUSING HISTORY
[ ] Current lease, if you have one
[ ] Landlord names, addresses, and phone numbers
[ ] Eviction notice, court summons, or judgment, if applicable
[ ] Shelter or homelessness verification letter, if applicable

HOUSEHOLD AND MEDICAL
[ ] Custody or guardianship documents, if applicable
[ ] Disability verification for accommodation requests
[ ] DD-214 or VA documentation, for veterans

TIPS
- Keep originals; give copies to programs and landlords.
- Store digital photos of every document in one secure folder.
- Ask each program which documents they require before your appointment.
