The Process

What to expect after you submit

  1. Step 1

    Submit a Housing Coordination Request

    Complete a short request online or by phone. No referral is required to get started.

  2. Step 2

    Our team reviews your information

    A coordinator reviews what you shared, confirms your service area, and identifies what to discuss first.

  3. Step 3

    A Housing Coordinator contacts you

    We reach out during regular office hours to talk through your situation, your goals, and any documents that may be needed.

  4. Step 4

    We begin identifying housing options and community resources

    Together we work through available housing opportunities and connect you with the community programs that fit your household.

Who We Work With

Who may request coordination

  • Individuals and families navigating a housing change or housing instability
  • Veterans and their households, including those working with VA or veteran organizations
  • Older adults seeking accessible or affordable housing options
  • Individuals living with disabilities who need accessible units or supportive services
  • Households referred by hospitals, agencies, or community organizations
  • Residents of Prince George's, Montgomery, and Anne Arundel counties, Maryland

Final eligibility for any specific housing opportunity or program is determined by that program's own requirements.

Helpful Documents

What to gather

  • Photo identification for all adult household members
  • Social Security cards, when available
  • Proof of income, benefits, or employment
  • Rental history or landlord references, when available
  • DD-214 or VA documentation for veteran households, when available
  • Documentation of disability or accessibility needs, if applicable

Do not wait until you have everything. Submit your request and your coordinator will help you work through what is missing.
